Privacy Policy: Mountain View Surgical Associates

Mountain View Surgical Associates (“MVSA,” “we,” “us”) respects your privacy. This policy explains how we collect, use and protect information you give us through this website (mtnviewsurgical.com), our consultation-request form and our text-message (SMS) program. It does not change your rights under HIPAA or our Notice of Privacy Practices, which governs your medical information. Information we collect through the website
When you use our consultation-request form, we may collect:
- Your name, mobile phone number and email address.
- The service you are interested in, your insurance (if you choose to tell us) and any message you write.
- Whether you ticked the box agreeing to receive text messages, and when.
- Basic technical information, such as your IP address and browser type, used only to prevent fraudulent or automated submissions.
- How you reached our site (for example, a search engine or an online ad), so we can see which of our outreach works.
How we use your information
- To respond to your request and contact you about the care you asked about.
- If you agreed to texts, to send you a text asking you to confirm your request, and one reply once you confirm.
- To protect patients and our practice from fraudulent or automated submissions.
- To keep our website and forms secure and working well.
We do not sell your personal information. We do not use information from the consultation-request form to send you marketing.
Text messages (SMS)
Program: Mountain View Surgical Associates Patient Notifications.
What we send: If you tick the text-message box on our consultation-request form, we send one text asking you to reply YES to confirm your request, and one reply once you confirm. If you are a patient and gave the practice permission to text you when you registered, you may also receive one text after a visit asking about your experience, no more than once every 90 days. We do not send marketing or promotional texts.
Message frequency: Message frequency varies. Usually up to 2 messages per consultation request, and at most 1 post-visit message every 90 days.
Costs: Message and data rates may apply, depending on your mobile plan.
Opting out: Reply STOP to any message to stop receiving texts. We will send one message confirming you have opted out and will send no further texts unless you opt in again by replying START. Reply HELP for help, or call us at 303-805-1855.
Consent is optional: You do not have to agree to texts to request a consultation or to receive care from us. If you leave the box unticked, we will still receive your request and contact you by phone or email.
Who sends the texts: Texts are sent on our behalf by our technology provider, Frontier Consulting LLC, which operates our website form and messaging systems under contract and may use your information only to provide that service to us.
Sharing of mobile information
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. We do not sell, rent or trade your mobile phone number or your text-messaging consent to anyone, including lead generators.
We share information only with service providers who help us run this website, our forms and our text messages (such as our website host, our firewall and anti-bot service, our technology provider and our text-message carrier). They process it only on our behalf, under contract, and not for their own marketing. We may also disclose information when the law requires it. All of the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
How we protect your information
Form submissions travel over an encrypted connection and are stored encrypted on a secured, single-purpose system with access controls, audit logging and firewall protection. Only authorized personnel can see them.
How long we keep website form information
We keep information from the consultation-request form only as long as we need it to respond to your request and for reasonable record-keeping, unless the law requires us to keep it longer.
Your choices
You do not have to give us information through the form. You can ask us to correct or delete information you submitted through the website form by calling 303-805-1855 or writing to us at the address below. Requests about your medical records are handled under our Notice of Privacy Practices and HIPAA, not this policy.
Children
This website is not directed to children under 13, and we do not knowingly collect their information through it. A parent or guardian should request care for a minor.
Changes to this policy
We may update this policy from time to time. The current version, with its effective date, is always the one posted on this page.
