These terms govern your use of the Mountain View Surgical Associates (“MVSA,” “we,” “us”) website, mtnviewsurgical.com, and our text-message (SMS) program. By using the site you agree to them.
Use of the site
This website gives general information about our practice, services and locations. It is not medical advice and does not replace professional care. Do not rely on it for medical decisions; contact us or another qualified provider directly. In an emergency, call 911.
Consultation requests
Information you submit through our forms is used to contact you about your request. Please give accurate information. Agreeing to receive text messages is optional and is not a condition of requesting a consultation or receiving care.
SMS terms and conditions
- Program name: Mountain View Surgical Associates Patient Notifications.
- Program description: If you tick the text-message box on our consultation-request form, we send one text asking you to reply YES to confirm your request, and one reply once you confirm. Patients who gave the practice permission to text them at registration may also receive one text after a visit asking about their experience, no more than once every 90 days. We do not send marketing or promotional texts.
- Message frequency: Message frequency varies. Usually up to 2 messages per consultation request, and at most 1 post-visit message every 90 days.
- Costs: Message and data rates may apply.
- Opt out: You can cancel the SMS service at any time. Reply STOP to any message. We will send one message confirming you have opted out, and then you will receive no further texts from us. To rejoin, reply START.
- Help: Reply HELP to any message, or call us at 303-805-1855.
- Carriers: Carriers are not liable for delayed or undelivered messages.
- Consent: Consent to receive text messages is not a condition of requesting a consultation, purchasing any goods or services, or receiving care.
- Sender: Texts are sent on our behalf by our technology provider, Frontier Consulting LLC.
- Privacy: No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. For details, see our Privacy Policy.
Acceptable use
You agree not to use the site or its forms to submit fraudulent, abusive or unauthorized content, or to try to disrupt or gain unauthorized access to the site. We use anti-fraud and rate-limiting measures and may block or decline submissions that appear automated or fraudulent.
Intellectual property
Site content is owned by or licensed to Mountain View Surgical Associates and may not be reproduced without permission, except as the law allows.
Limitation of liability
To the fullest extent the law allows, Mountain View Surgical Associates is not liable for damages arising from your use of, or inability to use, this website. This does not affect rights you may have under applicable law.
